BLISS CARES is a 501c-3 nonprofit organization that has helped over 2,000 patients and has provided more than 3.8 million dollars in STD treatment and prevention in the past 2 years. For each dollar you donate, 98¢ goes towards healthcare costs for underserved individuals.

Fill Out Your Application
You’ll receive a text message with a financial assessment form to fill out. You’ll be able to send the form back via text message, along with photos of the following documents:
1. Proof of income.
If you receive any of the following types of income, send us a photo of your most recent:
- Pay stubs
- Tax return
- W2
- Unemployment benefits
- Social security disability
- Social security retirement
- VA pension
- Supplemental security income

2. Proof of residence in Florida.
This could be a driver’s license, Florida ID, or a piece of mail showing your address (such as a utility bill or a bank statement).
We’ll contact you to let you know whether your application was approved, and what level of coverage you’ll receive.
